Please note that this form is for housing-related complaints only. Any complaint submitted through this form will not be considered a written grievance disputing an action or decision of the Tribal Member Benefits Department related to applications for Housing Assistance Programs in accordance with Part T of the Match-E-Be-Nash-She-Wish Band of Pottawatomi Indians Housing Assistance Handbook (“Handbook”). If you would like to file a grievance related to a Tribal Member Benefits Department action or decision, you must file using the method and format required by Part T of the Handbook.
